Transcatheter closure of patent foramen ovale (PFO) has been widely accepted as a non-invasive alternative to medical therapy. Recently, many series have reported a high incidence of residual shunts in patients with presumed device closure of PFO. This study was aimed at assessing the safety and efficacy of the Amplatzer septal occluder (ASO) in transcatheter closure of PFO. From January 2001 to December 2003, 54 patients (23 males, 31 females, mean age 29.4 +/- 8.4, range: 14-45 years) with stroke or transient ischemic attack were studied. The subjects were followed up clinically and by transthoracic and transesophageal echocardiography. The study population consisted of 38 patients with cryptogenic stroke and 16 patients with paradoxical embolism. Based on transthoracic and transesophageal echocardiography, 11 patients had transitory right-to-left shunt after percutaneous closure of PFO. Eight patients (3 with paradoxical embolism and 5 with cryptogenic stroke) had at least 1 recurrence of ischemic stroke in 3 months to 2 years of follow-up. The device was successful in the complete closure of PFO in 52 patients, but there were 2 inadequate closures. There was no residual shunt after percutaneous closure of PFO in patients who received transcatheter PFO closure without major contraindication. ASO closure can be performed in all types of PFO, with complete device closure in more than 80% of patients. However, ischemic events recur in about 20% of patients after transcatheter closure. Patients with contraindications for long-term anticoagulant therapy for the prevention of thromboembolic events should be monitored closely. A
