A KD incidence cohort was defined in the province of Pordenone, Northern Italy, and the hospital discharge records from January 1, 1999 to December 31, 2000 were reviewed. Case definition was based on the presence of fever, skin changes, and at least 4 of the clinical criteria for KD. Total population of the province was estimated. During the study period, 675 patients were hospitalized for KD in the province of Pordenone, with an incidence of 117/100,000 person-years. The mean annual incidence was 117/100,000 population, with a clear bimodal age distribution. The crude annual incidence was 25 cases/100,000 under 5 years of age and 64 cases/100,000 between 5 and 14 years of age. The male:female ratio was 1.3:1. In total, 516 (75.7%) patients met the above diagnostic criteria for KD. No death occurred in the study period. Eighty-eight patients (13.2%) required coronary angiography. After a mean follow-up of 2.2 years, a further 11 patients developed coronary artery aneurysms, giving an overall incidence of 14.1/100,000. Seven patients (1%) required coronary revascularization for ischemic heart disease during the follow-up period. The mean total follow-up time was 4.5 years (range 1 month-11 years). KD in our setting has an annual incidence of about 75/100,000 children, with a bimodal age distribution.
