The combination of traditional diagnostic procedures with a rapid laboratory method to identify strictly anaerobic bacteria, in particular Bacteroides fragilis and Prevotella, is described. A double selective medium was prepared and used in conjunction with the ready-to-use Vitek anaerobe identification system (bioMérieux, Marcy l’Etoile, France) for the identification of anaerobes. In a retrospective study, the results of the anaerobe identification system were compared with those of the phenotypic method (KBD agar and pre-reduced Biocollecta-Groningen) as the reference method. A total of 945 isolates of anaerobes recovered from clinical specimens of 830 patients were tested. Of these, 803 strains were identified with the anaerobe identification system, and of these 796 (96.5%) were correctly identified. These data were confirmed by the comparison of the anaerobe identification system with the phenotypic method for 338 strains in a prospective study. In this comparison, 302 strains were identified with both methods, whereas in 25 cases the anaerobe identification system failed and in 13 cases it was the phenotypic method that failed. With regard to the anaerobe identification system, only one isolated strain was misidentified and in five cases the test was not performed. This study shows that the anaerobe identification system can be a reliable alternative to the phenotypic method for the routine identification of strictly anaerobic bacteria in the clinical laboratory.
